If you have not already, please complete a 506 form and return it to the SUSD Native American Indian Center - NAIC by mail or email.

1. Fill out the 506 Form. Find the form online at NAIC506 Form or contact the NAIC at (209) 933-7040 ext. 8083 for a physical copy.
2. Submit the completed 506 Form and proof of tribal membership to Gina Lopez, NAIC Specialist, at gmplopez@stocktonusd.net. You may also physically mail it to the following address: Native American Indian Center 100 W. Dr. Martin Luther King Jr. BLVD Portable 105 Stockton CA, 95206
3. You will receive a welcome confirmation email. The 506 Form remains on file until the student's 12th-grade year, so no need to fill one out if you already have one on file.
