Flu
- Everyone 6 months & older should receive a yearly flu vaccine.
- SUSD works with San Joaquin County Public Health to provide FREE Flu Vaccines to SUSD students, families, children 2 years and up, and staff.

- SUSD Flu consent forms are sent home with students in early Fall every year. These consent forms are not included in the Back-to-School packet.
- If you would like your child to receive the Flu vaccine, please see your school site Health Services staff OR the links below.

- Call the Health Services Department at (209) 933-7060 to schedule your appointment
- This is open to all SUSD District Staff, Students, Family and Community Members
- Complete the Flu Consent Form before you arrive – or we will have forms available in the office for you
