Referral Process & Contacts

If your child is between ages 3 and 5 and you suspect a disability, please call us at (209) 933-7345 to initiate the referral process. Our Preschool Assessment Team will guide you through consultation and evaluation.

Step 1

Call our office to initiate the referral. SUSD staff will complete the referral form for your child. Referral Form

Step 2

A phone or in-person consultation/assessment will be scheduled with the Preschool Assessment Team.

Step 3

Bring or send required documentation: proof of residency, birth certificate, parent ID/driver’s license, and any educational/psychological reports.

Step 4

Evaluation of your child. Parents must consent to special education evaluations by signing the Assessment Plan.

Step 5

IEP meeting to determine eligibility and placement for services.

Timeline

Referral to Consent

Within 15 calendar days of receiving a referral, the school will contact the parent to schedule a social history. Consent should be obtained within 30 calendar days. Summer referrals will follow up when school resumes.

Consent to Evaluation

The school district has 60 calendar days from receiving parent consent to complete special education evaluations.

Consent to Placement

Within 60 school days from consent, the district completes evaluations, holds the IEP meeting, and offers the Least Restrictive Environment (LRE) placement for your child.
