Parent / Guardian
- Photo ID, such as driver’s license, passport, or employee ID
- Proof of address, such as lease, mortgage, utility bill, or government document
Child’s Age Verification
- Birth certificate or passport
- County registrar statement
- Affidavit, if applicable
Immunizations
- Required for school entry
- Tdap booster for grades 7–12
- View immunization requirements
Additional Documents
- Prior school records
- IEP, if applicable
- Guardianship or custody documents
- Interdistrict Transfer approval, if applicable
- High school transcripts, if applicable

Native American Students
Complete the ED 506 Form and return it to the Enrollment Office or Native American Center.
