Form 1095 – Health Coverage Information
The following forms provide information about your health insurance coverage for the 2025 benefit plan year.
Form 1095-C
Form 1095-C was mailed to employees on February 9, 2026.
This form shows information about the health insurance coverage offered by the District and whether you were enrolled in coverage during 2025.

Form 1095-C may help when preparing your tax return, but it is not required to file your taxes.

If you have not received your Form 1095-C, please contact the Benefits Department:
Form 1095-B
Form 1095-B is sent directly by your health insurance carrier, such as:
- Kaiser Permanente
- Anthem
- Blue Shield
- UnitedHealthcare
This form provides information regarding the months during 2025 in which you and your dependents were enrolled in health coverage.
