Form 1095 – Health Coverage Information

The following forms provide information about your health insurance coverage for the 2024 Benefit Plan Year:

• Form 1095-C: This form was mailed to employees on February 24, 2025. It shows information about the health insurance coverage offered by the District and whether you were enrolled in coverage during 2024. While this form can help when preparing your individual income tax return, it is not required to file your taxes.
  If you have not received your 1095-C form, contact the Benefits Department at 209-933-7026 or email benefits@stocktonusd.net to request a copy.
• Form 1095-B: This form will be sent directly to employees by their health insurance carrier (for example, Kaiser, Anthem, Blue Shield, or UnitedHealthcare). It provides details on which months in 2024 both you and your dependents were enrolled in coverage.
  If you have not received this form by February 1, 2025, please contact your medical insurance carrier directly (phone number on the back of your medical ID card).
