For the Child You Are Enrolling
- Birth Certificate
- Current Immunization Record
- Medical Insurance Card
- Current IEP/IFSP, if applicable
Income for Parent/Guardian
Please provide income documentation for the past 12 months.
- Employment: Recent Tax/W-2 or two recent pay stubs
- Other Income: Verification of Benefits, such as CalWORKS, CalFresh, Foster Agreement, or SSI documentation
