Claim For Damages

If you experience damage to personal or District property, or if you are involved in an incident that results in damages, you can file a Claim for Damages through the Risk Management Department. The claim process helps ensure that damages are documented, assessed, and addressed in a timely manner.
Types of Claims for Damages Include:
- Damage to District property (e.g., vehicles, buildings, equipment)
- Damage to personal property on District premises (e.g., personal belongings damaged during school activities)
- Incidents involving liability (e.g., accidents or injuries involving District property or operations)
How to File a Claim for Damages:

Report the Incident to your supervisor immediately, including all details related to the damage or incident.

Complete the Claim for Damages Form: (Be sure to include)
- Date, time, and location of the incident
- Description of the damage or incident
- Names and contact information of any witnesses (if applicable)
- Photos or documentation of the damage (if available)
- Police Report if applicable

Submit the Claim Form to Risk Management:
- Send the completed form to Rodrigo Lemus-Chavez
- CC: Adelita Valadez and Claudia Agnos for processing.

Investigation and Resolution:
Risk Management will review the claim, investigate the incident, and determine the appropriate next steps. This may involve further documentation or communication with the parties involved. -
Follow-Up:
Once the claim is processed, you will be notified of the resolution and any actions that may be taken to address the damage.
Important Notes:
- Filing a claim does not guarantee reimbursement for damages; all claims will be reviewed on a case-by-case basis.
- Claims for damages may require additional information or documentation depending on the nature of the incident.
