You can also find forms needed for your child and Immunization requirements needed for school entry. If you are an SUSD employee, you can find information on CPR certification classes and TB clinics.
Our mission is to diligently pursue through all avenues the education, training, and services that will lead to each student's attainment of optimal health to achieve his/her highest potential as a learner.
