The Human Resources Department of the Stockton Unified School District is required by state law, Education Code and Board Policy to provide employees with the following annual notifications and requires a signed acknowledgment that such notifications were received and read.

- It must be completed by March 1, 2020.
- Print your name and employee ID number, sign, date and return to the Human Resources Department, attention Mary Aguilar to acknowledge receipt of the above notifications. This document will be placed in your personnel file. Note: If you complete this form electronically, nothing further is required.
