About the PFT Documentation:

California requires participation for 5th, 7th, and 9th graders in all 5 of the fitness zones to be documented. Collection forms are optional to help record your student data. Illuminate input is required to receive a participation rate.

Collection Forms

Collection forms are used to record data (raw scores to give to students and participation data to be recorded per the state requirement). Below are 3 optional forms for your convenience. Click on the link to go to the form.

• Illuminate Class Collection Form 
  • Form Type: Illuminate, download as PDF
  • Description: This form automatically populates your students' information including name and ID number. Once printed, you can record all 5 participation or raw score metrics.
• Blank Class Collection Form
  • Form Type: Google Sheet
  • Description: The blank class collection form allows you to write (or type) in your students' names and ID numbers as well as their participation and/or score information for each of the 5 tests.
• Blank Individual Collection Form 
  • Form Type: PDF
  • Description: The individual collection form is to record individual student information for each of the 5 required tests.

Illuminate Participation Report

The Physical Fitness Participation Rates per grade and exercise are included in the School Accountability Report Card (SARC). To verify your participation rates, here is a summary report available in Illuminate once you enter your students' participation information:
