8th grade athletes going to Franklin, here is information you will need for sports:

FREE Sports Physicals for Franklin athletes!
Edison High School is hosting FREE sports physical clinics this month.
May 25   12:30-3:30

Edison Health Center / 1425 S. Center Street / South Side of Edison High School Campus near the Tennis Courts and Preschool
New to SUSD Athletics?  Haven’t had a physical in 12 months?  You need a sports physical before you can start conditioning for any Fall sports team! 
Call 209-444-8300 to make an appointment. 
Students must wear masks and complete the Qualtrics survey before coming to campus.  Social distancing will be enforced.  
Parents need to complete both of these forms.

Students will need to bring these signed forms to their appointment OR parents are welcome to accompany their child to the appointment and fill out the forms on site.
